PAST TRAINING VERIFICATION REQUESTS

Training Verification - Completed within Five Years Post Training Program Completion

Training completed within five years should be directed to the training program for verification.
Please reference the following list of Residency and Fellowship Programs to identify the appropriate department contact.

Training Verification Completed Five Years Post Training Program Completion

Standard Training Verification Requests:
The University of California, Irvine, Office of Graduate Medical Education (GME), requires an official verification request letter, accompanied by an Authorization of Release of Information form, in order to complete a training verification. All verification training requests must be submitted to the University by a verification company. 

It is the policy of the University of California, Irvine, Office of Graduate Medical Education (GME), to verify only, the dates and type of post-graduate (Residency or Fellowship) training for all training completed more than five (5) years ago. The Office of GME, or individual training programs, can only verify that a trainee was deemed competent for the years of training credit awarded based on the training program requirements at that time.

Please follow the instructions outlined below to submit your request. Email submissions should be sent to GME Registrar Sonia Portillo-Mendoza

  • Submit your request including name, email and contact number along with a copy of your verification request form and authorization of release of information form. Both are required to process a standard verification request.
  • The GME Office charges $50 per verification request (per Verification Company/Board). Fees will be collected by the GME Registrar once a request has been submitted.
  • Please allow 2-3 weeks for processing.

Board Application Training Verification Requests:
If you are requesting that a training verification be completed as part of a medical board application, please complete the Medical Board Verification of Training Request Form. This form must be notarized and mailed directly to the Office of Graduate Medical Education at the address provided below. 

A Program Director that signs Medical Board forms does so because your State requires a signature to license the physician applicant. However, the Program Director signing the licensing form is not responsible for the information stated. It is important to note that the training information submitted on the form, per the University verification process, is from GME’s Resident/Fellowship file and not from the individual training program.

Please follow the instructions outlined below to submit your request. Email submissions should be sent to GME Registrar Sonia Portillo-Mendoza

  • Submit your request including name, email and contact number along with a copy of your medical board verification request document(s).
    • Please complete and sign any sections of the medical board form(s) that require your attention; this varies per state. 
  • Please mail your notarized Medical Board Verification of Training Request Form to the GME Office at 333 City Boulevard West, Suite 870, Orange, CA 92868.
  • The GME Office charges $50 per verification request (per Verification Company/Board). Fees will be collected by the GME Registrar once a request has been submitted.
  • Please allow 2-3 weeks for processing.
